Abstract
Objective: To compare clinical outcomes of surgical mitral valve replacement (SMVR) and TEER with MitraClip therapy in patients with secondary mitral regurgitation.
Methods: We performed a review of the literature of primary research comparing clinical outcomes of surgical mitral valve replacement (SMVR) and TEER with MitraClip therapy in patients with secondary mitral regurgitation. A preliminary search of the medical database PubMed was conducted in October 2024 using the terms mitral valve, secondary mitral regurgitation, transcatheter edge-to-edge repair, surgical candidacy, and comparative outcomes. Limits included “published in the last 15 years, humans, English, randomized controlled trials, and cohort studies.”
Results: After performing the search of the literature, three studies were selected. These studies included “MitraClip therapy and surgical edge-to-edge repair in patients with severe left ventricular dysfunction and secondary mitral regurgitation: mid-term results of a single-centre experience” 1 by De Bonis et al which was a prospective cohort study comparing efficacy and safety endpoints for those treated with TEER vs. surgery at four years. Search result selection also included “Percutaneous repair or surgery for mitral regurgitation,” 2 by Feldman et al which was a randomized controlled trial studying safety and efficacy endpoints at one year between groups treated with TEER and with surgery. Lastly, the literature selection included “Five-year outcomes of transcatheter reduction of significant mitral regurgitation in high-surgical-risk patients” 3 by Kar et al. This was a prospective study on the TEER group included in Feldman et al study, looking at the safety and efficacy endpoints after five years from treatment.
Conclusion: Though TEER with MitraClip placement is not yet a competitive alternative to surgery, it can be an effective option for patients who would not be good candidates for surgical replacement
